Caring for You throughout Your Hospital Stay

When you’re admitted to Saratoga Hospital, you’re assured of the best possible care from the Hospitalist, Saratoga Inpatient Physicians (SIP), members of a private, professional practice who specialize in treating hospitalized patients. That means we’re experienced not just in meeting your medical needs, but also in addressing your concerns as a hospital patient. And because we never see patients outside the hospital, we’re available when and where you need us, throughout your hospital stay.

Answering Your Questions

Q: What is a hospitalist?
A: A hospitalist is a physician who specializes in caring for patients who are hospitalized. We do not have a private outpatient medical practice, only a private inpatient practice for hospitalized patients. Most Hospitalists are trained as general, internal medicine physicians or family practice physicians. Your family physician voluntarily contracts with SIP to provide for all the hospitalized care of his or her patients. SIP and your family physician work together to share information so your care is optimized. SIP provides high quality, inpatient medical care, 24 hours a day, seven days a week, throughout your hospital stay.

Q: Will my family physician know what’s happening with me?
A: Yes. On the day you enter the hospital, we will notify your family doctor that you’ve been admitted and the reason why. SIP will also provide your doctor with your detailed history, including your diagnosis and our plan for your care during your hospitalization. SIP will communicate with your family physician throughout your stay. On the day you leave the hospital SIP will also notify your family physician. Your doctor will also receive a written record of all tests, treatments, medications, and consultations ordered during your hospital stay. If follow-up care is necessary, this will be indicated as well.

Q: How do I get up-to-date information on my care?
A: If you or your family have questions, ask the hospitalist. He or she will have the latest information on your progress and our plans for your continued care. If you need to contact the hospitalist, there are three ways to do so: 1. Ask your nurse to contact SIP for you. 2. Call SIP’s business office between 9 am and 5 pm at (518) 587-1141 and the staff will assist you; or 3. If those hours are inconvenient, call the Saratoga Hospital operator at (518) 587-3222 and ask that they assist you to reach the hospitalist.

Q: What will happen once I leave the hospital?
A: Before you leave the hospital, SIP will arrange for any support services – such as home care, physical or occupational therapy – that you may need. We also will provide any necessary referrals for hospice or short- or long-term care. Your hospitalist also will make sure you have the prescriptions and medications you need, and will arrange for any tests that should be done before your follow-up visit with your family doctor. Once you leave the hospital your family physician will again take charge of your care.

Q: If I don’t have a family physician, who will continue my care when I am discharged from the hospital?
A: Your hospitalist will provide you with information on family physicians in the area and will help you arrange for a follow-up visit with the doctor of your choice.
